The following forms are required for your player to participate with Liberty FC:

2023/2024 Player Contract 

2023/2024 HS Commitment Contract - for high school players only
This contract is for high school players not playing in the 2023 fall season. It is to hold a roster spot for you in the 2023/2024 winter and 2024 spring seasons. The $200.00 deposit will be credited, once paid, to your 2023/2024 winter fees.

Copy of a US Birth Certificate or a US passport/US Certificate if player is not a US citizen - For NEW PLAYERS ONLY in order to be registered with US Club Soccer and the leagues Liberty FC plays in. Players cannot be put on a leagues official roster or obtain a pass card without one of these documents.
