The following forms are required for your player to participate with Liberty FC:
Medical Release Form
Lindsay Law Form
US Club Soccer Form - not needed if you have played for Liberty FC starting with the Fall of 2022 season
2024/2025 Player Contract
Copy of a US Birth Certificate or a US passport/US Certificate if player is not a US citizen - For NEW PLAYERS ONLY in order to be registered with US Club Soccer and the leagues Liberty FC plays in. Players cannot be put on a leagues official roster or obtain a pass card without one of these documents.
